Background Twenty-three years after Hepler and Strand published their well-known definition of Pharmaceutical Care (PhC), confusion remains about what the term includes and how to differentiate it from other terms. In February 2011, the MoH issued the Long-Term Medical and Health Personnel Development Plan (2011–2020),76 which projects that the number of Chinese pharmacists will reach 550,000 by 2015 and 850,000 by 2020. Pharmaceutical care The pharmaceutical care is defined as “The responsible provision of drug therapy for the purpose of achieving definite therapeutic outcomes that improve the patients quality of life” These outcomes are : Cure of the disease Elimination or reduction of patient`s symptomology Arresting or slowing of a disease process It was developed by examining some key source documents on pharmaceutical care and medication therapy management. In this patient-centered and outcome-oriented practice, the pharmacist assumes direct responsibility for all the patient's drug-related needs and is held accountable for this commitment (Cipolle, Strand, & Morley, 1998). Fourteen members of PCNE and 10 additional experts attended the moderated discussion. The definition of pharmaceutical care, its historical development in practice, as well as its impact on patient care and health outcomes have been extensively discussed elsewhere. Second, the social need is met by the practitioners' responsibility to identify, resolve, and prevent drug therapy problems. Further, it was proposed that the discipline of clinical pharmacy must be involved in research in order to contribute to the generation of new knowledge for the advancement of health outcomes and quality of life (American College of Clinical Pharmacy, 2008; Hepler, 1987, 2004). While some believe that the practice of pharmaceutical care is a sole responsibility of the pharmacists, others believe that it is a shared responsibility between all healthcare providers (Allemann et al., 2014). Hence, pharmaceutical care is a revolutionary concept and forms the basis of pharmacy practice as clearly outlined in the evolving mission of pharmacy practice (Commission to Implement Change in Pharmaceutical Education, 1993b).
